ABSTRACT

Objective@#To investigate the clinical efficacy and effects of periodontal endoscope (PE)-assisted subgingival scaling and root planning (SRP) and traditional SRP on the psychological and quality of life of patients with periodontitis.@*Methods@#This study was reviewed and approved by the Ethics Committee, and informed consent was obtained from the patients. Patients with periodontitis who were treated in the Department of Periodontology, Nanjing Stomatological Hospital, Medical School of Nanjing University from April 2018 to December 2022 with residual periodontal pockets (PD ≥ 5 mm) 6 weeks after traditional SRP treatment were enrolled, and the residual periodontal pockets were further treated with PE-assisted SRP (PE+SRP). After 6 weeks of traditional SRP treatment and 3 months of PE+SRP treatment, clinical indicators, including plaque index (PLI), probing depth (PD), clinical attachment loss (CAL) and bleeding on probing (BOP), were measured, and periodontal tissue self-awareness scale scores, oral health impact profile-14 (OHIP-14) score and dental fear scale (DFS) score were collected. Moreover, visual analog scale (VAS) scores were collected after traditional SRP and PE-assisted SRP treatments.@*Results@#Twenty-three patients with periodontitis, including 832 sites of 486 affected teeth, were included in the clinical study. Three months after PE+SRP treatment, all clinical periodontal indicators, PLI (t = 9.254, P<0.001), PD (t = 50.724, P<0.001), CAL (t = 22.407, P<0.001) and BOP (t = 9.217, P<0.001), were significantly improved. Compared with traditional SRP (VAS: 2.48 ± 1.70), the pain caused by PE+SRP (VAS: 2.57±1.80) was not significantly different (t = 0,192, P = 0.850). There was no significant difference in the scores of the periodontal tissue self-awareness scale between the two groups (t = 1.485, P = 0.152). The OHIP-14 (SRP: 12.13±7.63; PE+SRP: 10.26 ± 5.25, t = -1.589, P = 0.126) and DFS (SRP: 40.70 ± 12.63; SRP+PE: 41.57 ± 12.61, t = 0.404, P = 0.690) scores were not significantly different.@*Conclusion@#All clinical periodontal indicators were significantly improved after PE-assisted SRP treatment of residual periodontal pockets, and compared with traditional SRP, PE-assisted SRP had no negative impact on the quality of life or psychological status of patients with periodontitis. Therefore, PE+SRP can be promoted in clinical practice.

ABSTRACT

As of April 30, 2020, cumulative confirmed coronavirus disease 2019 (COVID-19) cases exceeded 3 million worldwide and 1 million in the US, with an estimated fatality rate of more than 7 percent. Because the occurrence patterns of new confirmed cases and deaths over time are complex and seemingly country-specific, estimating the long-term pandemic spread is challenging. I developed a simple transformation algorithm to investigate the characteristics of the case and death time series per nation and described the universal similarities observed in the transformed time series of 19 nations in the Group of Twenty (G20). A transformation algorithm of the time series data sets was developed with open-source software programs to investigate the universal similarities among the cumulative profiles of confirmed cases and deaths of 19 individual nations in the G20. The algorithm extracted and analyzed statistical information from daily updated COVID-19 pandemic data sets from the European Centre for Disease Prevention and Control (ECDC). Two new parameters for each nation were suggested as factors for time-shifting and time-scaling to define reduced time, which was used to quantify the degree of universal similarities among nations. After the cumulative confirmed case and death profiles of a nation were transformed by using reduced time, most of the 19 nations, with few exceptions, had transformed profiles that closely converged to those of Italy after the onset of cases and deaths. The initial profiles of the cumulative confirmed cases per nation universally showed 3 to 4-week latency periods, during which the total number of cases remained at approximately ten. The latency period of the cumulative number of deaths was approximately half the latency number of cumulative cases, and subsequent uncontrollable increases in human deaths seemed unavoidable because the coronavirus had already widely spread. Immediate governmental actions, including responsive public-health policymaking and enforcement, are observed to be critical to minimize (and possibly stop) further infections and subsequent deaths. In the pandemic spread of infectious viral diseases, such as COVID-19, studied in this work, different nations show dissimilar and seemingly uncorrelated time series profiles of infected cases and deaths. After these statistical phenomena were viewed as identical events occurring at a distinct rate in each country, the reported algorithm of the data transformation using the reduced time revealed a nation-independent, universal profile (especially initial periods of the pandemic spread) from which a nation-specific, predictive estimation could be made and used to assist in immediate public-health policymaking.

ABSTRACT

@#Chronic periodontitis is a chronic inflammatory disease caused by plaque microorganisms, and removal of plaque and calculus is the gold standard for nonsurgical periodontal treatment. However, complete debridement is difficult, especially in some complex anatomical sites. Excessive scaling may result in the loss of healthy cementum and lead to dental hypersensitivity. Studies have shown that a diode laser can exhibit the best performance in an environment with blood because its wavelengths (630-1 064 nm) are close to the absorption peaks of heme and melanin and they have broad application prospects in the oral field. In nonsurgical periodontal treatment, diode lasers have three treatment modes: soft diode laser, antimicrobial photodynamic therapy and low-level laser therapy, which can be used alone or in combination. Although diode lasers cannot replace mechanical treatment to remove calculus, they can remove infected periodontal pocket epithelium, change the microcirculation to promote wound healing, reduce bleeding and relieve pain through photothermal effects and biological stimulation. The effect of diode laser treatment depends on the treatment dose. It is necessary to precisely control the output intensity and control the irradiation time to avoid thermal damage to the tissue. In the future, extensive research at the molecular level is needed to reveal the tissue response. At the same time, more high-quality, large-sample randomized controlled trials are needed to standardize the use of lasers for different stages and grades of periodontitis.

ABSTRACT

Interval timing is involved in a variety of cognitive behaviors such as associative learning and decision-making. While it has been shown that time estimation is adaptive to the temporal context, it remains unclear how interval timing behavior is influenced by recent trial history. Here we found that, in mice trained to perform a licking-based interval timing task, a decrease of inter-reinforcement interval in the previous trial rapidly shifted the time of anticipatory licking earlier. Optogenetic inactivation of the anterior lateral motor cortex (ALM), but not the medial prefrontal cortex, for a short time before reward delivery caused a decrease in the peak time of anticipatory licking in the next trial. Electrophysiological recordings from the ALM showed that the response profiles preceded by short and long inter-reinforcement intervals exhibited task-engagement-dependent temporal scaling. Thus, interval timing is adaptive to recent experience of the temporal interval, and ALM activity during time estimation reflects recent experience of interval.

ABSTRACT

ABSTRACT Body mass is known to affect muscle strength and the outcome of some functional tests, so that heavier and taller people will be stronger than lighter and smaller ones. Ratio standard (RS) has been widely used to remove the body mass effect, despite long date criticism due to its inadequacy. Allometry (ALLO), in turn, has been applied as an efficient method for normalizing muscular strength. As the bench press (BP) is a well-recognized strength and conditioning exercise for older adults, the aim of the present study was to verify the influence of body mass on the performance assessment of a group of older men in the BP, by comparing the absolute, RS and ALLO approaches. Sixteen healthy old men (65.5±5.13 years old; 75.42±9.78Kg; 1.73±5.98m; 25.11±2.71 kg/m2; 24.76±4.10 %fat) volunteered to participate in the study. Maximum dynamic load was verified by individual one-repetition maximum (1-RM) tests. Comparisons of means revealed that significant 1-RM difference between lighter (54.9±8.85Kg) and heavier (66.2±8.86Kg) participants was identified only in absolute approach (p<0.05; ES=0.57). RS failed in completely remove the body mass effect, allowing correlation between normalized muscular strength and BM (r=0.23), in contraire of ALLO (r=0.03 and 0.06). Kendall's concordance coefficient revealed an absolute lack of agreement between approaches when compared their respective ordinal classifications (kw=0.003; p>0.05). In line with previous research, ALLO has shown to be the only suitable method to remove adequately the body mass effect and to provide appropriated performance scores for the older men evaluated in this study.


RESUMO Sabe-se que a massa corporal afeta a força muscular e o resultado de alguns testes funcionais, de modo que pessoas mais pesadas e altas serão mais fortes que as mais leves e menores. A razão-padrão (RP) tem sido largamente utilizada para remover o efeito da massa corporal, apesar de críticas conhecidas há muito tempo devido sua inadequação. Alometria (ALO), do contrário, tem sido aplicada como um método eficiente para normalizar a força muscular. Como o supino é um exercício de força e condicionamento bem reconhecido para idosos, o objetivo deste estudo foi verificar a influência da massa corporal sobre a avaliação do desempenho de um grupo de idosos no supino, comparando as abordagens absoluta (AB), RP e ALO. Dezesseis idosos saudáveis (65,5±5,13 anos de idade; 75,42±9,78Kg; 1,73±5,98m; 25,11±2,71 kg/m2; 24,76±4,10 %gordura) se voluntariaram para participar no estudo. A máxima carga dinâmica foi verificada pelos testes de 1 repetição máxima (1RM). Na comparação das médias, a diferença significante do 1RM entre os participantes leves (54,9±8,85Kg) e pesados (66,2±8,86Kg) foi identificada apenas na abordagem ABS (p<0,05; ES=0,57). A RP falhou em remover completamente o efeito da massa corporal, permitindo correlação entre a força muscular e a massa corporal normalizadas (r=0,23), ao contrário da ALO (r=0,23 e 0,06). O coeficiente de Kendall revelou ausência de concordância entre as abordagens quando comparadas as suas respectivas classificações ordinais (kw=0,003; p>0,05). Em linha com pesquisas anteriores, ALO tem se mostrado como o único método viável para remover adequadamente o efeito da MC e para oferecer escores de desempenho mais apropriados para homens idosos, como os avaliados por este estudo.

ABSTRACT

Fundamento: existen escasos estudios que evalúen la aplicación subgingival de antimicrobianos en el tratamiento de la periodontitis del adulto. Objetivo: evaluar la efectividad de antimicrobianos como coadyuvantes del raspado y alisado radicular en el tratamiento de la periodontitis leve y moderada del adulto. Métodos: estudio cuasiexperimental de intervención terapéutica, realizado en la Clínica Estomatológica de Santiago de Cuba, entre noviembre 2018 y septiembre 2021, sobre 144 pacientes con diagnóstico de periodontitis crónica leve y moderada. Mediante el método probabilístico aleatorio simple, se conformaron tres grupos con 48 integrantes, de forma voluntaria. El grupo control tratado con el raspado y alisado radicular y dos grupos de estudio en que, además, se aplicó una concentración subgingival de tetraciclina y azitromicina, respectivamente. Los pacientes fueron examinados y evaluados a los siete días, al mes y tres meses. Se empleó el análisis de varianza ANOVA para evaluar la igualdad de medias y la prueba ji-cuadrado para la efectividad de curación con un 95 % de confiabilidad. Resultados: hubo reducción de profundidad de sondaje y mayor ganancia de inserción clínica en los tres grupos, pero a favor de los grupos de estudio. Todas las terapias fueron efectivas, pero obtuvo mayor curación el grupo dos (RAR + Tetraciclina) con 97,9 %. Conclusiones: las aplicaciones subgingivales de tetraciclina y azitromicina resultaron efectivas como coadyuvantes del raspado y alisado radicular en el tratamiento de la periodontitis leve y moderada del adulto; con la tetraciclina se logró mayor mejoramiento de parámetros clínicos, disminución de profundidad de sondaje, ganancia de inserción clínica y menor tiempo de curación.


Background: there are few studies that evaluate the subgingival application of antimicrobials in adult periodontitis. Objective: to evaluate the effectiveness of antimicrobials as contributing for scaling and root planing in the treatment of mild and moderate periodontitis in adults. Methods: quasi-experimental study of therapeutic intervention, carried out at the Santiago de Cuba Stomatological Clinic, between November 2018 and September 2021, on 144 patients diagnosed with mild and moderate chronic periodontitis. Using the simple random probabilistic method, three groups with 48 members were formed voluntarily. The control group treated with scaling and root planing and two study groups in which, in addition, a subgingival concentration of tetracycline and azithromycin, respectively, was applied. The patients were examined and evaluated at seven days, one month and three months. ANOVA analysis of variance was used to evaluate the equality of means and the chi-square test for the effectiveness of healing with 95% reliability. Results: there was a reduction in probing depth and greater gain in clinical attachment in the three groups, but in favor of the study groups. All therapies were effective, but group two (RAR + Tetracycline) obtained a greater cure with 97.9%. Conclusions: subgingival applications of tetracycline and azithromycin were effective as adjuvants for scaling and root planing in mild and moderate periodontitis in adults; Tetracycline achieved greater improvement in clinical parameters, decreased probing depth, gain in clinical attachment, and shorter healing time.

OBJECTIVES@#To investigate the genetic polymorphism of InDel loci in SifalnDel 45plex system in the Han population in Jiangsu Province and the Mongolian population in Inner Mongolia, and to evaluate the effectiveness of the system in forensic medicine.@*METHODS@#SifaInDel 45plex system was used for genotyping in blood samples of 398 unrelated individuals from the above two populations, and allele frequencies and population genetic parameters of the two populations were calculated respectively. Eight intercontinental populations in the gnomAD database were used as reference populations. The genetic distances between the two studied populations and eight reference populations were calculated based on the allele frequencies of 27 autosomal-InDels (A-InDels). The phylogenetic trees and multidimensional scaling (MDS) analysis diagrams were constructed accordingly.@*RESULTS@#Among two studied populations, the 27 A-InDels and 16 X-InDels showed no linkage disequilibrium between each other and the allele frequency distributions were in Hardy-Weinberg equilibrium. The CDP of the 27 A-InDels in two studied populations were all higher than 0.999 999 999 9, and the CPEtrio were all less than 0.999 9. The CDP of the 16 X-InDels in Han in Jiangsu and Mongolian in Inner Mongolia female and male samples were 0.999 997 962, 0.999 998 389, and 0.999 818 940, 0.999 856 063, respectively. The CMECtrio were all less than 0.999 9. The results of population genetics showed that the Jiangsu Han nationality, Inner Mongolia Mongolian nationality and East Asian population clustered into one branch, showing closer genetic relationship. The other 7 intercontinental populations clustered into another group. And the above 3 populations displayed distant genetic relationships with the other 7 intercontinental populations.@*CONCLUSIONS@#The InDels in the SifaInDel 45plex system have good genetic polymorphism in the two studied populations, which can be used for forensic individual identification or as an effective complement for paternity identification, and to distinguish different intercontinental populations.

ABSTRACT

Objective @#To evaluate the clinical effect of endoscopic-assisted subgingival scaling and root planning (SRP) in the treatment of periodontitis. @*Methods@#PubMed, EMBASE, The Cochrane Library, Web of Science, CNKI, WanFang Data, and VIP databases were searched for randomized controlled trials (RCTs) related to endoscopy-assisted SRP. The search time limit was from the establishment of the database to September 15, 2021. The outcome indicators included in the study included the plaque index (PLI), probing depth (PD), attachment loss (AL), and bleeding index (BI). Review Manager 5.4 and Stata 12.0 software were used for the meta-analysis.@* Results@#A total of 111 studies were retrieved, and 5 quantitative studies were included after screening. Meta-analysis showed that for sites with 4 mm ≤ PD < 6 mm, 3 and 6 months after treatment, there was no significant difference in the PD value between the endoscope assisted group and the simple SRP group (P > 0.05); for sites with PD ≥ 6 mm, the PD value of the endoscope assisted group was smaller than that of the simple SRP group 3 and 6 months after treatment. The difference between the two groups was statistically significant (P < 0.05), but there was no significant difference in PLI, Al or BI between the two groups (P > 0.05). @*Conclusion@#Compared with simple SRP, the auxiliary use of endoscopy has a better effect on reducing PD in deep periodontal pockets (PD ≥ 6 mm). However, for clinical indicators such as PLI, AL, and BI, there was no difference between the therapeutic effects of the two methods.

ABSTRACT

OBJECTIVE@#To compare the clinical efficacy of initial periodontal therapy in periodontitis patients with or without type 2 diabetes mellitus and its correlation with white blood cell counts.@*METHODS@#In this study, 32 chronic periodontitis patients without systemic disease (CP group) and 27 chronic periodontitis patients with type 2 diabetes mellitus (CP+DM group) were enrolled. At admission, all the patients went through periodontal examination and fasting blood examination(baseline). Probing depth (PD), attachment loss (AL), bleeding index (BI), plaque index (PLI), white blood cells (WBC) counts and fasting blood glucose (FBG) were recorded respectively, while hemoglobin A1c (HbA1c) was recorded only in CP+DM group. After that, initial periodontal therapy was performed. All the tests were repeated 3 and 6 months after treatment. The changes of periodontal clinical indexes and WBC levels were compared between the two groups before and after treatment, and the correlation between WBC and periodontal clinical indexes and glucose metabolism indexes were analyzed by generalized linear mixed model.@*RESULTS@#At baseline, the periodontal inflammation and destruction were similar in CP and CP+DM group, but the WBC level was significantly higher in CP+DM groups [(6.01±1.26)×109/L vs. (7.14±1.99)×109/L, P=0.01]. After 3 and 6 months of initial periodontal therapy, the mean PD, AL, BI, and PLI in CP+DM and CP groups were significantly lower than the baseline, and the PD in CP+DM group was further decreased by 6 months compared with 3 months [(3.33±0.62) mm vs. (3.61±0.60) mm, P < 0.05]. However, none of these periodontal indexes showed significant difference between the two groups by 3 or 6 months. In CP+DM group, HbA1c at 3 months and 6 months were significantly lower than the baseline [(7.09±0.79)% vs. (7.64±1.16)%, P < 0.05; (7.06±0.78)% vs. (7.64±1.16)%, P < 0.05], and FBG was significantly lower than the baseline by 6 months [(7.35±1.14) mmol/L vs. (8.40±1.43) mmol/L, P < 0.05]. The WBC level in CP group was significantly lower than the baseline level by 3 months [(5.35±1.37)×109/L vs. (6.01±1.26)×109/L, P < 0.05], while that in CP+DM group was significantly lower than the baseline level by 6 months [(6.00±1.37)×109/L vs. (7.14±1.99)×109/L, P < 0.05]. The analysis of genera-lized linear mixed model showed that WBC level was significantly positively correlated with PD and FBG (P < 0.05).@*CONCLUSION@#Initial periodontal therapy can effectively improve the periodontal clinical status of patients with or without type 2 diabetes mellitus, and have benefits on glycemic control in diabetic patients. However, the response of periodontal indexes and WBC level to initial therapy were relatively delayed in diabetic patients. WBC plays an important role in the correlation between diabetes mellitus and periodontitis.

Background: World Health Organization (WHO) advocates use of weight bands in antiretroviral therapy (ART) guidelines. Allometric scaling could be a more reliable method because it uses a non-linear approach in relating dose to body weight. This study evaluates performance of the allometric ¾ power model in comparison to WHO weight band method in children receiving ART. Methods: Records of children receiving (ABC/3TC) + DTG were reviewed. Paediatric ABC/3TC dose was calculated from the adult dose using the allometric ¾ power model and compared to WHO weight band dose. Results: WHO weight band strategy grouped 50.6% of the children in the 25 kg category and therefore received the adult dose of ABC/3TC (600 mg/300 mg); only 1.1% received this dose with allometric scaling. Mean dose (3.8 tablets) for the WHO weight band dosing method was found to be significantly higher (p<0.0001) than for allometric scaling (1.5 tablets). Conclusions: WHO weight bands may result in the 25 kg weight category receiving a much higher dose leading to ADRs. Using allometric scaling, we recommend a weight band strategy that could improve paediatric ABC/3TC dosing

ABSTRACT Objective: The purpose of this study was to clinically evaluate the effect of one-stage full-mouth ultrasonic debridement in the treatment of diabetic subjects with severe chronic periodontitis. Methods: Sixteen patients with uncontrolled diabetes (glycated hemoglobin ≥ 7%) and severe chronic periodontitis (at least 8 teeth with a probing depth and bleeding on probing were selected and randomly assigned to 2 treatment groups: control group (n=7) - quadrant-wise scaling and root planning; test group (n=9) - full-mouth ultrasonic debridement in one session of 45 minutes. The following outcomes were assessed: plaque index, gingival index, bleeding on probing, probing depth, clinical attachment level and gingival margin position. All parameters were assessed at baseline, 1 and 3 months after treatment. Probing depth, clinical attachment level and gingival margin position were analyzed using ANOVA and Tukey test, and the other clinical parameters analysed with the Friedman and Mann-Whitney tests. Results: An improvement in probing depth, clinical attachment level was observed after treatment in both groups, but without statistically significant differences between them. After 3 months, in initially deep pockets, the PD decreased 1.6 mm and 1.7 mm in test and control group, respectively. Clinical attachment level gain ≥ 2 mm was statistically significant at sites in control (10.1%) and test (13,4%) groups in 3 months. Conclusions: One-stage full-mouth ultrasonic debridement promoted similar clinical outcomes to those obtained with the scaling and root planning in the treatment of diabetic subjects with severe chronic periodontitis.


RESUMO Objetivo: Avaliar clinicamente o efeito do debridamento ultrassônico de boca toda no tratamento da periodontite crônica severa em pacientes diabéticos. Métodos: Dezesseis pacientes diabéticos descompensados (Hemoglobina Glicada ≥ 7%) com periodontite crônica severa (mínimo de 8 dentes com profundidade de sondagem ≥ 5 mm e sangramento à sondagem) separados aleatoriamente em 2 grupos: Controle (7): raspagem e alisamento radicular por quadrante e Teste (9): debridamento ultrassônico sessão única de 45 minutos. Os parâmetros clínicos avaliados foram: Índice de placa, Índice gengival, Sangramento à Sondagem, Profundidade de Sondagem, Nível de inserção clínico e Posição da Margem Gengival. As variáveis foram avaliadas no início, 1 e 3 meses após o tratamento. Para análise das variáveis quantitativas (Profundidade de sondagem, Nível de inserção e Posição da margem gengival de bolsas moderadas) foram realizados ANOVA e teste de Tukey e para as demais foram utilizados os testes de Friedman e Mann-Whitney. Resultados: Observou-se melhora na profundidade de sondagem e nível de inserção clínica após o tratamento em ambos os grupos, sem diferenças estatisticamente significativas entre eles. Após 3 meses, nas bolsas inicialmente profundas, a profundidade de sondagem reduziu 1,6 e 1,7 mm nos grupos teste e controle, respectivamente. Houve um ganho estatisticamente significante no nível de inserção ≥ 2 mm nos grupos controle (10,1%) e teste (13,4%), aos 3 meses. Conclusão: O debridamento ultrassônico de boca toda promoveu resultados clínicos semelhantes aos obtidos com a raspagem e alisamento radicular no tratamento da periodontite crônica severa em indivíduos diabéticos.

ABSTRACT

Objective: This randomized controlled trial aimed to compare nanohydroxyapatite with fluoride on managing post ultrasonic scaling Dentine hypersensitivity (DH). Material and Methods: Thirty patients (aged 20-50 years) with post ultrasonic-scaling DH were included in this study. The sample was randomly divided into three equal groups of 60 teeth each: the first group received nanohydroxyapatite material, the second group received fluoride material and the third group received sterile water as a placebo (controls). The materials were applied once for each patient. All patients were instructed to rate the level of pain before treatment, and after 1 hour, 24 hours, 2 weeks and 1 month on the numerical rating scale (NRS). The Kruskal-Wallis test, Mann-Whitney tests, linear regression analysis were used for the statistical analysis. Significance level was set at 0.05. Results: Both nanohydroxyapatite and fluoride were successful in reducing pain associated with DH when compared with the placebo in subsequent follow-ups (p < 0.05). However, one-hour and one-day post application, nanohydroxyapatite could reduce hypersensitivity pain moreeffectively than fluoride (p < 0.05). Conclusion: Nanohydroxyapatite material was found to besignificantly more effective in reducing the DH that followed ultrasonic scaling one-hour and one-day post application as compared to fluoride and sterile water. Both fluoride and nanohydroxyapatite had similar effect on DH after two-weeks and one-month after application. (AU)


Objetivo: Este ensaio clínico randomizado teve como objetivo comparar a nano-hidroxiapatita com o flúor no manejo da hipersensibilidade dentinária (HD) pós-raspagem ultrassônica. Material e Métodos: Trinta pacientes (com idades entre 20-50 anos) com HD pós-raspagem ultrassônica foram incluídos neste estudo. A amostra foi dividida aleatoriamente em três grupos iguais com 60 dentes cada: o primeiro grupo recebeu material de nano-hidroxiapatita, o segundo grupo recebeu material de flúor e o terceiro grupo recebeu água esterilizada como placebo (controle). Os materiais foram aplicados uma vez para cada paciente. Todos os pacientes foram instruídos a avaliar o nível de dor antes do tratamento, e após 1 hora, 24 horas, 2 semanas e 1 mês na escala de avaliação numérica (NRS). Os testes de Kruskal-Wallis, Mann-Whitney e análise de regressão linear foram usados para a análise estatística. O nível de significância foi estabelecido em 0,05. Resultados: Tanto a nano- hidroxiapatita quanto o flúor foram bem-sucedidos na redução da dor associada à HD quando comparados ao placebo em acompanhamentos subsequentes (p <0,05). No entanto, 1 hora e 1 dia após a aplicação, a nano-hidroxiapatita pode reduzir a dor de hipersensibilidade de forma mais eficaz do que o flúor (p <0,05). Conclusão: O material da nano-hidroxiapatita foi significativamente mais eficaz na redução da HD que se seguiu à raspagem ultrassônica, 1 hora e 1 dia após a aplicação, em comparação com o flúor e a água estéril. Tanto o flúor, quanto a nano-hidroxiapatita apresentou efeito semelhante na HD após 2 semanas e 1 mês após a aplicação (AU)

ABSTRACT Absolute muscle strength or adjusted by body mass index (BMI) are useful to identify sarcopenia. However, these values are not accurate for older adults with extreme body sizes because the nonlinear relationship between strength and body size. The purpose was to determine cut-off points for identify sarcopenia in older adults using allometric coefficients to normalise handgrip strength (HGS) and 30-sec chair stand test (30-s CST) by body size. Allometric exponents were proposed with log-linear models for body-size variables (body mass, height and BMI). The remotion of body-size effect on muscle strength with allometric normalisation was tested by partial correlation. Cut-off points for low muscle strength were established by ROC curve and Youden index considering functional limitation (six-minute walk test<400m). Allometric exponents provided for body-size variables range from -0.01 to 2.28 (HGS) and -0.27 to 0.21 (30-s CST). The effect of body size on muscle strength was removed with allometric normalisation (r<0.30). Cut-off points accuracy was always adequate (AUC≥0.78; p<0.001). In conclusion, cut-off points of HGS and 30-s CST allometrically normalised were proposed to identify sarcopenia in Portuguese older adults and allometry maintained adequate the accuracy (AUC>70%). Allometry removed influence of body size on the expression of HGS and 30-s CST and permits evaluate muscle strength regardless of body-size.


RESUMO A força muscular absoluta ou ajustada pelo índice de massa corporal (IMC) é útil para identificar a sarcopenia. No entanto, esses valores não são precisos para idosos com tamanhos corporais extremos devido à relação não linear entre força e tamanho corporal. O objetivo foi determinar os pontos de corte para identificar a sarcopenia em idosos usando coeficientes alométricos para normalizar a força de preensão manual (FPM) e teste de sentar e levantar da cadeira em 30 segundos (30-s CST) por tamanho corporal. Os expoentes alométricos foram propostos com modelos log-lineares para variáveis de tamanho corporal (massa corporal, estatura e IMC). A remoção do efeito do tamanho corporal na força muscular com normalização alométrica foi testada por correlação parcial. Os pontos de corte para baixa força muscular foram estabelecidos pela curva ROC e índice de Youden considerando a limitação funcional (teste de caminhada de seis minutos<400m). Os expoentes alométricos fornecidos para variáveis de tamanho corporal variam de -0,01 a 2,28 (FPM) e -0,27 a 0,21 (30-s CST). O efeito do tamanho corporal na força muscular foi removido com normalização alométrica (r<0,30). A precisão dos pontos de corte sempre foi adequada (AUC≥0,78; p<0,001). Em conclusão, foram propostos pontos de corte para FPM e 30-s CST normalizados alometricamente para identificar sarcopenia em idosos portugueses e a alometria manteve a precisão adequada (AUC>70%). A alometria removeu a influência do tamanho corporal na expressão da FPM e 30-s CST e permite avaliar a força muscular independentemente do tamanho corporal.

ABSTRACT@#Erectile dysfunction (ED) and periodontitis have common risk factors, such as diabetes mellitus and tobacco smoking. Multiple reports are available in regard to the association between ED and chronic periodontitis (CP). The study aimed to determine the association of ED and CP in selected Malaysian population. In this study, 74 patients (mean age = 52.4 ± 10.9 years old) diagnosed with ED, from scores via the International Index of Erectile Function (IIEF-5) questionnaire, were included. ED severity was classified as mild, mild to moderate, moderate, and severe. Periodontal condition was recorded using basic periodontal examination (BPE) method, of which scores of 0, 1, 2, and 3 were associated with having no periodontitis while a score of 4 was considered to have periodontitis. There are 40 (54.1%) subjects found to have periodontitis and the association of ED and periodontitis showed a moderate positive degree of correlation, ρ = 0.487 (p < 0.001). The percentage of subjects having periodontitis indicated an increasing trend with the severity of ED; from 19.0% (mild ED), 54.2% (mild to moderate ED), 75.0% (moderate ED), to 84.6% (severe ED). A greater degree of correlation was noted between dental scaling experience and ED, ρ = 0.635 (p < 0.001). Binomial logistic regression had shown no other co-morbidities and factors were affecting this relation. There seemed to be an association between ED and periodontitis existing in these selected Malaysian populations.

Background:Periodontitis is multifactorial disease which is initiated with plaque formation that will initiate an inflammatory response which can cause destruction of tissues & tooth supporting structure. If left untreated it will cause gingival recession and bone destruction which will lead to tooth mobility. Sometimes it is difficult to eliminate periodontal pathogens completely from deepest areas of periodontal pockets, to overcome these issues photodynamic therapy (PDT) was used as it is local non-invasive treatment modality without any side effects. Aim:The aim of present study was to investigate the effectiveness of Photodynamic Therapy (PDT) as an Adjunct To Scaling & Root Planing in the managementOf Periodontal Disease. Material And Methods:Literature was searched systemically and studies were identified based on the-PICO (Glossary of Evidence Based Terms 2007). Electronic database search of Pubmed, Google scholar, Medline and scopus was performed using (MESH) terms-Photodynamictherapy,periodontitis, microbiological assessment. Articles published between year 2009-2019 were reviewed. Patient treated with SRP alone on one side and other side with SRP+ PDT. Recording of Clinical parameters like PPD,CAL,BOP,PS,GR were done from baseline to 3,6 month. Microbiological and biochemical analysis were also evaluated to check level of RANKL/OPG, IL-1?, TNF-?, Aggregatibacter actenomycetocomitans, Porphyromonas gingivalis, Tanerella forsythia Conclusion::A large evidence suggest that PDT when used along with SRP resulted in significantimprovement in clinical parameters.Significant improvement in biochemical parameters such as IL-1? , Tnf-? , RANKL/OPG were also seen after periodontal therapy along with PDT when compared with SRP alone.

Context: The impact of periodontal disease during pregnancy and its effect on adverse pregnancy outcomes is seen in the literature. When it comes to the link of disease related to periodontium to that of adverse pregnancy outcomes, a need can arise if a significant cause-effect relationship does exist or not between them. Aim: The study was aimed to determine the association of periodontal health status in pregnant women with the occurrence of preterm low birth weight (LBW) infants in Vadodara, Gujarat. Settings and Design: An interventional study with 100 patients was conducted, of which 67 participants were included in the control group and 33 participants were included in the intervention group. A total of 12 participants dropped out from the study and 88 were analyzed for the outcome. Subjects and Methods: The Community periodontal Index of Treatment needs index was taken for all enrolled participants and then were divided into interventional group and control group. Participants in the interventional group underwent scaling and root planning. Data related to the time of delivery and weight of the baby was taken from the hospital records. Statistical Analysis: A comparison of baseline characteristics was made using unpaired t-test. Chi-square test was used for the analysis of intergroup comparison. The odds ratio and the relative risk calculation were also done. P ? 0.05 was considered for statistical significance. Results: The odds ratio for both preterm and LBW were 3.86 times and 2.96, respectively. The Chi-square statistical test analysis was statistically significant for both preterm and LBW infants on the intergroup comparison. Conclusion: Periodontal disease can be considered as one of the risk factors for preterm LBW babies as not only the presence of disease condition causes an increase in inflammatory mediator but also the elimination of the disease condition reduces the adverse pregnancy outcomes.

RESUMEN Introducción: El tratamiento periodontal no quirúrgico corresponde a la terapia estándar para la periodontitis crónica. En este contexto, se ha propuesto que el uso de probióticos orales como complemento al tratamiento estándar podría mejorar los resultados obtenidos clínicamente, sin embargo no existe claridad respecto a su efectividad. Métodos: Se realizó una búsqueda en Epistemonikos: base de datos de revisiones sistemáticas en salud mantenida mediante el cribado de múltiples fuentes de información, la cuál incluye MEDLINE, EMBASE, Cochrane, entre otras. Se extrajeron los datos desde las revisiones identificadas y un análisis de los datos de los estudios primarios. Se realizó un metanálisis y se preparó una tabla de resumen de los resultados utilizando el método GRADE. Resultados y conclusiones: Se Identificaron seis revisiones sistemáticas que en conjunto incluyeron 14 estudios primarios, todos correspondientes a ensayos aleatorizados. De los resultados analizados se concluye que el uso de probióticos orales como complemento al tratamiento no quirúrgico de periodontitis crónica probablemente resulta en nula o poca diferencia en la reducción de la profundidad de sondaje, el sangrado al sondaje y una ganancia de nivel de inserción clínico en los sitios afectados. Por otro lado, no se encontraron estudios que evaluaran el riesgo de presentar halitosis en pacientes sometidos a terapia periodontal no quirúrgica con adición de probióticos.


ABSTRACT: Introduction: Non-surgical periodontal treatment is the standard treatment to chronic periodontitis. It has been proposed that the use of oral probiotics as an adjunctive therapy may improve clinical outcomes, however there is no clarity regarding its effectiveness. Methods: We searched in Epistemonikos, the largest database of systematic reviews in health, which is maintained by screening multiple information sources, including MEDLINE, EMBASE, Cochrane, among others. We extracted data from the systematic reviews, reanalyzed data of primary studies, conducted a meta-analysis and generated a summary of findings table using the GRADE approach. Results and conclusions: We identified six systematic reviews including 14 studies overall, of which all were randomized trials. We concluded that the addition of probiotics to non-surgical periodontal treatment probably results in little or no difference in probing depth reduction, clinical attachment level gain and reduction in bleeding on probing. No studies were found evaluating halitosis in patients undergoing non-surgical periodontal treatment with the addition of probiotics.

ABSTRACT

Se presenta el caso clínico de una paciente mujer de 39 años con diagnóstico de obesidad clase 1 y periodontitis generalizada que acudió al posgrado de Periodoncia para reducir el "sangrado de sus encías". Al examen clínico se evidenció inflamación generalizada del margen gingival y papilas interdentales, profundidad de sondaje promedio de 3,8 mm, pérdida de inserción clínica promedio de 3,9 mm y lesiones de furcación clase 1. Se planificó un tratamiento periodontal mecánico evidenciándose mejora en los parámetros clínicos al cabo de seis meses, sin embargo, persistieron las bolsas periodontales por lo que se indicó terapia periodontal quirúrgica resectiva a través de colgajos de Widman modificado. Con un seguimiento de dos años se evidenció mantenimiento de la salud periodontal con una profundidad al sondaje media de 2 mm sangrado al sondaje de 6% e índice de higiene oral de 11%, así también presentar zonas con profundidad de sondaje >3 mm pero sin sangrado al sondaje en dichas zonas. Se concluye que luego de una terapia periodontal quirúrgica resectiva, los parámetros periodontales de inflamación gingival, profundidad al sondajes y sangrado al sondaje fueron reducidos y controlados en una paciente diagnosticada con obesidad clase 1 y periodontitis generalizada.


Clinical case: 39-year-old female patient with class 1 obesity and generalized periodontitis diagnosis, attended the Postgraduate Periodontology program to reduce "bleeding from her gums". The clinical examination revealed generalized inflammation of the gingival margin and interdental papillae, an average probing depth of 3.8 mm, an average clinical attachment loss of 3.9 mm and class 1 furcation lesions. A mechanical periodontal treatment was planned, showing improvement in the clinical parameters after six months, however, the periodontal pockets persisted, for which resective surgical periodontal therapy was indicated through modified Widman flaps. With a two-year follow-up, periodontal health was maintained with a mean probing depth of 2 mm, bleed-ing on probing of 6% and oral hygiene index of 11%, as well as presenting areas with probing depth> 3 mm but without bleeding on probing in these areas. It is concluded that after resective surgical periodontal therapy, the periodontal parameters of gingival inflammation, depth to probing and bleeding on probing were reduced and controlled in a patient diagnosed with class 1 obesity and generalized periodontitis.

Abstract Current study assessed the impact of Lantana camara invasion on native plant diversity in Pothohar region of Pakistan. The approach used for study was random samplings and comparisons of diversity indices [number of species (S), abundance (N), species richness (R), evenness (Jꞌ), Shannon diversity index (Hꞌ) and Simpson index of dominance (λ)] with two categorical factors i.e., invaded and non-invaded (control). Control plots harboured by an average of 1.74 more species/10m2. The control category was diverse (Hꞌ=2.56) than invaded category (Hꞌ=1.56). The higher value of species richness in control plots shows heterogeneous nature of communities and vice versa in invaded plots. At multivariate scale, ordination (nMDS) and ANOSIM showed significant magnitude of differences between invaded and control plots at all sites. The decrease in studied diversity indices in invaded over control sites indicated that plant communities become less productive due to Lantana invasion.


Resumo O presente estudo avaliou o impacto da invasão de Lantana camara na diversidade de plantas nativas na região de Pothohar, no Paquistão. A abordagem utilizada para o estudo foram a amostragem aleatória e a comparação de índices de diversidade, como número de espécies (S), abundância (N), riqueza de espécies (R), equitabilidade (Jꞌ), índice de diversidade de Shannon (Hꞌ) e índice de dominância de Simpson (λ), com dois fatores categóricos, ou seja, invadidos e não invadidos (controle). As parcelas não invadidas tinham, em média, 1,74 espécie a mais / 10 m2 que parcelas invadidas. A categoria controle foi mais diversa (Hꞌ = 2,56) do que a categoria invadida (Hꞌ = 1,56). O maior valor da riqueza de espécies em parcelas de controle mostra a natureza heterogênea das comunidades, e vice-versa, em parcelas invadidas. Na escala multivariada, ordenação (nMDS) e ANOSIM mostraram magnitude significativa das diferenças entre as parcelas invadidas e controle em todos os locais. A diminuição nos índices de diversidade estudados em locais invadidos por controle indicou que as comunidades de plantas se tornam menos produtivas por causa da invasão de Lantana.

ABSTRACT

Objective To propose a quick and low-cost personalized diabetic foot modeling and insole design scheme, so as to reduce the plantar pressure accurately. Methods The foot model of the patient was constructed by scaling the model with foot feature parameters, to make biomechanical analysis on plantar pressure. By means of numerical mapping model of insole elasticity and plantar pressure, the three-dimensional (3D) personalized insole model with gradient modulus was constructed. The insole was then manufactured via 3D printing technology and used for experimental validation. Results The related mechanical parameters from finite element prediction of the foot model constructed by the scaling modeling method were close to those of the CT reconstructed model, and the maximum error was controlled within 15%. Compared with wearing the normal insole, the peak pressure of the personalized insole was effectively reduced by 20%. The time and economic cost of this simplified design was reduced by approximately 90%. Conclusions The design scheme of the diabetes insole shortens the design cycle, and the personalized insole can effectively and accurately reduce the sole pressure, and reduce the risk of foot ulcer, which provides a technical basis for the promotion of the personalized diabetes insole.
